Your time is valuable to us. To expedite your visit and ensure your time is spent with the doctor and not paperwork, please download the three forms in PDF below. Please complete the forms prior to arrival and bring them with you during your initial visit.
Please also bring all pertinent insurance information. If your insurance requires a referral and we have not received it from your primary care physician, your insurance will not allow the doctor to see you.
Please note that co-pays, coinsurances and deductibles are determined by your insurance company and are your responsibility. These amounts are not determined by our office.
We also kindly request all fees to be paid at the time of your visit. There is a $25 billing charge for fees not paid at the time of visit. For your convenience, we accept check, Visa, MasterCard and Discover.
